This study addresses the question how income affects health care utilization by the population aged 50 and over in the United States and a number of European countries with varying health care systems. The probabilities that individuals receive several medical services (visits to general practitioner, specialist, dentist, inpatient, or outpatient services) are analyzed separately using probit models. In addition to controls for income and demographic characteristics, controls for health status (both subjective and objective measures of health) are used. We analyze how the relationship between income and health care utilization varies across countries and relate these cross country differences to characteristics of the health care system, i. e., per capita total and public expenditure on health care, gate-keeping for specialist care, and copayments.

ABC of Multimorbidity is the first title to provide primary care practitioners with a practical approach to the complex issues of treating and managing patients with more than one morbidity. Ageing populations and earlier diagnosis of chronic conditions mean more people are living longer with multimorbidity. However, treatment guidelines are often designed for treatment in isolation of other morbidities. Multimorbidity management therefore requires a more patient centred approach and greater knowledge and coordination of existing services. Effective multimorbidity management both improves overall patient well-being and reduces the overall demand on health services. ABC of Multimorbidity examines how multimorbidities can be addressed within primary care, from the GP and family physician consultation through to the effective use of a range of health care services. It addresses complex issues such as polypharmacy, mental health, patient safety, patient involvement in self-management, and the role of the practitioner. It then provides guidance on how multimorbidities can be best treated and managed within primary care through specific interventions to improve outcomes. From an international, primary care editor and contributor team, ABC of Multimorbidity is a practical resource for general practitioners, family physicians, practice and specialist nurses, and others caring for multimorbid patients. It is also relevant for junior doctors, medical trainees and students.

What types of mental health care are right for the 21st century—in all countries, around the world? What is the best balance of primary care and specialist mental health care for people with mental illness? What is the best evidence for effective treatments and services? Which methods are most suitable for answering these questions? These are the tough issues addressed by this book. Leading experts from across the world bring their experience to bear in a series of chapters that set out the very best evidence in this field. They present clear accounts of what is known, extensively referenced, with critical appraisals of the strength of the evidence and the robustness of the conclusions that can be drawn. This book was inspired by the work of Michele Tansella on the development of community care globally. It comprises four sections, identifying the challenges associated with providing mental health services in high-, middle- and low-income countries, then describing ways to meet these challenges. A section on new research methods to produce practical evidence is a novel aspect of the book. The final section addresses how best to deliver new forms of care, bearing in mind the human resources available. The chapters extensively cite specific studies to enhance the practical relevance. Much of the cited research involves service users so their voice is heard throughout the text. Mental health is rapidly becoming recognised as one of the leading global health challenges. This book adds to our knowledge of the challenge and the solutions and stands to make a significant contribution to global mental health.

Written by the President and CEO of the Institute for Healthcare Improvement (IHI) and a leading health care journalist, this groundbreaking book examines how leading organizations in the United States are pursuing the Triple Aim—improving the individual experience of care, improving the health of populations, and reducing the per capita cost of care. Even with major steps forward – including the Affordable Care Act and the creation of the Center for Medicare and Medicaid Innovation – the national health care debate is too often poisoned by negativity. A quieter, more thoughtful, and vastly more constructive conversation continues among health care leaders and professionals throughout the country. Innovative solutions are being designed and implemented at the local level, and countless health care organizations are demonstrating breakthrough remedies to some of the toughest and most expensive challenges in health care. Pursuing the Triple Aim shares compelling stories that are emerging in locations ranging from Pittsburgh to Seattle, from Boston to Oakland, focused on topics including improving quality and lowering costs in primary care; setting challenging goals to control chronic disease with notable outcomes; leveraging employer buying power to improve quality, reduce waste, and drive down cost; paying for care under an innovative contract that compensates for quality rather than quantity; and much more. The authors describe these innovations in detail, and show the way toward a health care system for the nation that improves the experience and quality of care while at the same time controlling costs. As the Triple Aim moves from being largely an aspirational framework to something that communities all across the US can implement and learn from, its potential to become a touchstone for the work ahead has never been greater. Pursuing the Triple Aim lays out the vision, the interventions, and promising examples of success.

This article raises the problem of equity in the health system in Switzerland. Three dimensions of the concept of equity are taken into consideration: the inequality in the financing of the health system, the inequality in the distribution of the state of good health, and, finally, the iniquity in the access to health care. Some methodological developments are presented as well as the results. In conclusion we observe that the state of good health does not depend strongly on income but that it exists some iniquity in the access to health services from specialists and that the income inequality is increasing due to the financing of the health system.

Dementia is increasingly and widely recognised as a serious health and social challenge, in the developed world as well as in the developing world. The need therefore to design and implement dementia care services of high quality is becoming more and more vital, particularly given the likelihood of ever increasing demand in a world, which likely sees resources at best remaining at current levels. Designing and Delivering Dementia Services describes current developments in the design and configuration of dementia services. It offers an informative and detailed overview of what constitutes high quality care, considering the circumstances patients and carers may find themselves in. For dementia to get the priority it deserves, a number of factors are important and the book charts the invaluable contributions of various Alzheimer's Associations and Societies: this provides a focus on dementia strategies and plans at national levels: the book reports on the state of affairs regarding such strategies and provides a unique insight into the process of how one of these was developed and implemented. Recognising the need to prove that service developments lead to a higher quality of care, increased productivity and increased efficiency, the book links the resulting picture to service-based research methodologies, with an emphasis on the strengths and limitations of that research. Contributions from 17 countries on 4 continents give an overview of the state of affairs across the world, paying attention to successful – and less successful – initiatives to improve dementia care. The book furthermore provides pragmatic approaches to ensure planning becomes reality, highlights the need for structured workforce development, education and training and describes the opportunities afforded by assistive technology. This book is of prime informative and practical value given that pressures on dementia services are projected to mount across the world against a backdrop of limited resources and expertise. Designing and Delivering Dementia Services Defines the problems involved in meeting an increasing demand for dementia care services in a poorer world Maps initiatives and developments in the design and configuration of these services in a variety of international settings Analyses these developments against the background of political and health economic circumstances Provides a road map of where health services should go in response to this growing challenge. The first book to define, analyse and map initiatives for dementia care services in a time of increasing demand and decreasing resources, this book is essential reading for commissioners, senior clinicians and service planners in health and social care. It will also be of interest to academic researchers involved in qualitative services research as well as quantitative health economic research, health and social care managers and those involved in workforce planning and development.

Cluster randomised trials are trials in which groups (or clusters) of individuals are randomly allocated to different forms of treatment. In health care, these trials often compare different ways of managing a disease or promoting healthy living, in contrast to conventional randomised trials which randomise individuals to different treatments, classically comparing new drugs with a placebo. They are increasingly common in health services research. This book addresses the statistical, practical, and ethical issues arising from allocating groups of individuals, or clusters, to different interventions. Key features: Guides readers through the stages of conducting a trial, from recruitment to reporting. Presents a wide range of examples with particular emphasis on trials in health services research and primary care, with both principles and techniques explained. Topics are specifically presented in the order in which investigators think about issues when they are designing a trial. Combines information on the latest developments in the field together with a practical guide to the design and implementation of cluster randomised trials. Explains principles and techniques through numerous examples including many from the authors own experience. Includes a wide range of references for those who wish to read further. This book is intended as a practical guide, written for researchers from the health professions including doctors, psychologists, and allied health professionals, as well as statisticians involved in the design, execution, analysis and reporting of cluster randomised trials. Those with a more general interest will find the plentiful examples illuminating.

There are wide inconsistencies between, and even within, countries in how community-orientated care is defined and interpreted. The analysis presented in this book take as a starting point an evidence-based balanced care model in which services are provided in community settings close to the populations served, with hospital stays being reduced as far as possible, usually located in acute wards in general hospitals. The surprising conclusion from the research is that the same problems arise in all countries, regardless of resource status, and thus the recommendations of this book apply to mental health provision everywhere. This book reviews the implementation of community-orientated care using the balanced care model. It summarizes the steps, obstacles and mistakes that have been encountered in the implementation of community mental health care worldwide and presents guidelines on how to avoid them. It proposes realistic and achievable recommendations for the development and implementation of community-orientated mental health care over the next ten years. These guidelines will be of practical use to psychiatrists and other mental health and public health practitioners at all levels worldwide, including policy makers, commissioners, funders, non-governmental organisations, service users and carers. A core message of the book is that the mental health sector will more powerfully advocate for better services in future through strong and unified alliances, especially with powerful representation from consumer/service user and carer groups. Community-orientated care draws on a wide range of practitioners, providers, care and support systems (both professional and non-professional), though particular components may play a larger or lesser role in different settings depending on the local context and the available resources, especially trained staff. Research by a WPA task force has demonstrated that most of the challenges are common and global, but with local variations. The book is therefore relevant to psychiatrists and mental health workers in developed countries who are trying to deliver better health care on reduced budgets and for those in the developing economies who are in the position to modernise their mental health care. It provides clear, concise guidance on policy and practice decisions, learning from what has and has not worked in regions in the world. The book contains many tables documenting the evidence, supported by an essential reference list, and a Key Points summary for each chapter. Highly Commended in the Psychiatry section of the 2012 BMA Book Awards.

Promising Care: How We Can Rescue Health Care by Improving It collects 16 speeches given over a period of 10 years by Donald M. Berwick, an internationally acclaimed champion of health care improvement throughout the course of his long and storied career as a physician, health care educator and policy expert, leader of the Institute for Healthcare Improvement (IHI), and administrator of the Centers for Medicare & Medicaid Services. These landmark speeches (including all of Berwick’s speeches delivered at IHI’s annual National Forum on Quality Improvement in Health Care from 2003 to 2012) clearly show why our medical systems don’t reliably contribute to our overall health. As a remedy he offers a vision for making our systems better – safer, more effective, more efficient, and more humane. Each of Berwick’s compelling speeches is preceded by a brief commentary by a prominent figure in health care, policy, or politics who has a unique connection to that particular speech. Contributors include such notables as Tom Daschle, Paul Batalden, and Lord Nigel Crisp. Their commentaries reflect on how it felt to hear the speech in the context in which it was delivered, and assess its relevance in today’s health care environment. Krisa Tailor The Patient Revolution. How Big Data and Analytics Are Transforming the Health Care Experience

In The Patient Revolution, author Krisa Tailor—a noted expert in health care innovation and management—explores, through the lens of design thinking, how information technology will take health care into the experience economy. In the experience economy, patients will shift to being empowered consumers who are active participants in their own care. Tailor explores this shift by creating a vision for a newly designed health care system that's focused on both sickness and wellness, and is driven by data and analytics. The new system seamlessly integrates health into our daily lives, and delivers care so uniquely personalized that no two people are provided identical treatments. Connected through data, everyone across the health care ecosystem, including clinicians, insurers, and researchers, will be able to meet individuals wherever they are in their health journey to reach the ultimate goal of keeping people healthy. The patient revolution has just begun and an exciting journey awaits us. Praise for the patient revolution «A full 50% of the US population has at least one chronic disease that requires ongoing monitoring and treatment. Our current health care system is woefully inadequate in providing these individuals with the treatment and support they need. This disparity can only be addressed through empowering patients to better care for themselves and giving providers better tools to care for their patients. Both of those solutions will require the development and application of novel technologies. In Krisa Tailor's book The Patient Revolution, a blueprint is articulated for how this could be achieved, culminating in a vision for a learning health system within 10 years.» —Ricky Bloomfield, MD, Director, Mobile Technology Strategy; Assistant Professor, Duke Medicine «In The Patient Revolution, Krisa Tailor astutely points out that 80% of health is impacted by factors outside of the health care system. Amazon unfortunately knows more about our patients than we do. The prescriptive analytics she describes will allow health care providers to use big data to optimize interventions at the level of the individual patient. The use of analytics will allow providers to improve quality, shape care coordination, and contain costs. Advanced analytics will lead to personalized care and ultimately empowered patients!» —Linda Butler, MD, Vice President of Medical Affairs/Chief Medical Officer/Chief Medical Information Officer, Rex Healthcare «The Patient Revolution provides a practical roadmap on how the industry can capture value by making health and care more personalized, anticipatory, and intuitive to patient needs.» —Ash Damle, CEO, Lumiata «Excellent read. For me, health care represents a unique economy—one focused on technology, but requiring a deep understanding of humanity. Ms. Tailor begins the exploration of how we provide care via the concepts of design thinking, asking how we might redesign care with an eye toward changing the experience. Major changes are occurring in the United States population and the nation's health care institutions and delivery systems. Significant disparities in health status exist across population groups. But the health care enterprise, with all its integrated and disparate parts, has been slow to respond. Written by three nationally known scholars and experts, Diversity and Cultural Competence in Health Care: A Systems Approach is designed to provide health care students and professionals with a clear understanding of foundations, philosophies, and processes that strengthen diversity management, inclusion, and culturally competent care delivery. Focusing on current practice and health care policy, including the recently passed Patient Protection and Affordable Care Act of 2010 (ACA), this textbook integrates strategic diversity management, self-reflective leadership, and the personal change process with culturally and linguistically appropriate care into a cohesive systems-oriented approach for health care professionals. The essentials of cultural competence and diversity management covered in this text will be helpful to a wide variety of students because they encompass principles and practices that can be realistically incorporated into the ongoing work of any health care field or organization. Each chapter contains learning objectives, summary, key terms, and review questions and activities designed to allow students to understand and explore concepts and practices identified throughout the text.

Social factors have a powerful influence on human health and longevity. Yet the social dimensions of health are often obscured in public discussions due to the overwhelming focus in health policy on medical care, individual-level risk factor research, and changing individual behaviours. Likewise, in philosophical approaches to health and social justice, the debates have largely focused on rationing problems in health care and on personal responsibility. However, a range of events over the past two decades such as the study of modern famines, the global experience of HIV/AIDS, the international women’s health movement, and the flourishing of social epidemiological research have drawn attention to the robust relationship between health and broad social arrangements. In Health Justice, Sridhar Venkatapuram takes up the problem of identifying what claims individuals have in regard to their health in modern societies and the globalized world. Recognizing the social bases of health and longevity, Venkatapuram extends the ‘Capabilities Approach’ of Amartya Sen and Martha Nussbaum into the domain of health and health sciences. In so doing, he formulates an inter-disciplinary argument that draws on the natural and social sciences as well as debates around social justice to argue for every human being’s moral entitlement to a capability to be healthy. An ambitious integration of the health sciences and the Capabilities Approach, Health Justice aims to provide a concrete ethical grounding for the human right to health, while advancing the field of health policy and placing health at the centre of social justice theory. With a foreword by Sir Michael Marmot, chair of the WHO Commission on the Social Determinants of Health.

She does an excellent job deconstructing this from the patient experience. I look forward to a hopeful follow-up directed at changing the provider culture.» —Alan Pitt, MD, Chief Medical Officer, Avizia «Whether you're a health care provider looking to gain an understanding of the health care landscape, a health data scientist, or a seasoned business pro, you'll come away with a deeper, nuanced understanding of today's evolving health care system with this book.
