Diabetic nephropathy (a micro-vascular complication) is the most common cause of end stage renal disease (ESRD). If untreated, 80% of people who have type-1 diabetes and microalbuminuria will progress to overt nephropathy whereas 20-40% of those with type-2 diabetes over a period of 15 years will progress. It is concluded that diabetic patients suffer more from oxidative stress. Oxidative stress is still higher in diabetic patients with nephropathy complication than patients without nephropathy.

Prevalence of type 2 Diabetes Mellitus has been increasing significantly in all countries. It is widely recognized that the most serious problem in DM arises from the insidiously developing vascular complications.This cross-sectional study, aimed to determine the level of serum phospholipids in Sudanese patients suffering from diabetes mellitus type 2.Fifty samples were collected from Almotkamil centre. The study included both sexe, 25 males (aged 45-70years), whereas, 25 were females (aged 45-68years). Venous blood (2.5 ml) were taken from each patient in lithium heparin container; serum was obtained in plain container, and the samples were analyzed for serum phospholipids levels using spectrophotometer. Statistical analysis of the results showed that, the means of serum phospholipids in diabetic patients was higher 275.8mg/dl, p.value 0.000) compared to the reference value of phospholipids levels.From the result of the present study, it is concluded that;plasma phospholipids levels represent a marker for accelerated atherosclerosis, particularly in type 2 diabetic patients. Thus, measurement of plasma phospholipids could be useful for stratification of cardiovascular risk.

The groupwise division of patients was as follows:Group A: Type 2 DM patients with Chronic Obstructive Pulmonary Disease (n=15)Group B: Type 2 DM patients with Asthma (n=15)Group C: A control group of healthy non diabetic, non smoking subjects with no chest complaints (n=15)The study shows that spirometry results in type 2 diabetes patients with COPD or asthma require confirmation with further pulmonary function tests in the form of DLCO and lung volume measurements with body plethysmography. This is in accordance with the ATS/ERS 2005 recommendations on interpretative strategies in pulmonary function testing. Type 2 diabetes mellitus (T2DM) is a chronic, progressive metabolic disease characterized by chronic hyperglycemia. Although its main physiological abnormalities are insulin resistance and impaired insulin secretion, the specific underlying determinants of these metabolic defects remain uncertain. There are complex interactions between genetic, epigenetic, environmental and behavioral factors that contribute to the development of diabetes. Non-pharmacological and pharmacological interventions have been used for diabetic management. Over the past few years, research has started to focus on the use of novel adjuvant drugs as antioxidants and anti-inflammatory drugs for better management, as it was revealed that both oxidative stress and inflammation play a critical role in the disease pathogenesis.Thus, the development of antidiabetic drugs that can reverse insulin resistance is a potential therapeutic target. Although antidiabetic drugs may be effective in improving glycemic control, they do not appear to be effective in entirely preventing the progression of pancreatic ß-cells damage mediated by chronic hyperglycemia-induced decline in intracellular antioxidants. Diabetes mellitus is one of the most common problems caused by a combination of insulin resistance and impaired insulin secretion by pancreatic B cells. Nowadays, there are many drugs to treat diabetes. Oral antidiabetic drugs are associated with side effects such as weight gain, hypoglycaemia, lactate acidosis and none of these drugs have been shown to stop the progressive decline of beta-cell function. Insulin treatment is expensive and troublesome for the patients, and also induces weight gain. Given this background, there is a need for new classes of blood-glucose-lowering drugs that minimise the risk of hypoglycaemia. Incretin hormones derivatives and dipeptidyl peptidase-4 inhibitors are the latest marketed development in this field. Incretin hormones (GLP-1 and GIP) amplify insulin secretion and are rapidly degraded by dipeptidyl peptidase-4, therefore two strategies have been pursued to exploit the beneficial actions of the hormone:(I) development of more stable activators of the GLP-1 receptor (so-called incretin) like liraglutide, taspoglutide and so on (II) inhibitors of DPP-4 like alogloptin, dutagliptin and so on. Platelet flow cytometry is an emerging tool in diagnostic and therapeutic hematology. It is eminently suited to assess the expression of platelet surface receptors both qualitatively as well as quantitatively. It can thus serve as a useful parameter for in vivo expression of platelet activation and thus fore-warn the risk of thromboembolism which is inherent in patients with untreated type 2 diabetes mellitus. This technique can also be used to study and compare the effect of various antiplatelet drugs on the level of platelet activation and to establish any dose-effect relationship of these drugs. Topographical localization of platelet granules and study of platelet-platelet and platelet-leukocyte interaction is also possible by this procedure. All these parameters serve as pointers towards the presence of activated platelets in the circulation with its thromboembolic consequences. This study was undertaken with the primary object of determining platelet reactivity in patients with untreated type 2 diabetes mellitus and to study the effect of clopidogrel administration on the state of in vivo platelet activation in these patients.

Diabetes mellitus (DM) is a world-wide endemic with increasing prevalence in both developing and developed countries in a common cause of disability and death all over the world .The outburst of diabetes in the word is mainly due to type 2 diabetes about (90 - 95%). Its specific etiology is not yet known its frequency varies in different racial and ethnic subgroups and is often associated with a strong familial, likely genetic predisposition. It was denoted that type2 DM was associated with certain HLA class II alleles were analyzed for their genotyping by Polymerase Chain Reaction-Sequences Specific Olegionucleotide (PCR-SSO) technique. The present study revealed that diabetic patients were positively associated with HLA-DQB1*0201 (83% vs. 5.0%) which is the most prevalent in patients followed by DRB1*1137 (46.7% vs. 0.0%); DRB1*0401 (41.7% vs. 2.5%),and DRB1*1306(15% vs.0.0%) while HLA,A*0201;B*3559;Cw*0410 and DQB1*0501 is negatively associated in type 2DM in comparison to healthy control groups. This study has shown that there is no significant association between FBS,HbA1c,serum insulin,HOMA2 β-function , HOMA2-IR ,serum adiponectin ,serum adenosine deaminase and HLA alleles .

The aim of this study in general practice was to identify and determine the microbiological profile and antibiotic susceptibility patterns of organisms isolated from the diabetic patients. 50 patients presented with one or more infections and the pus fluid was collected from these patients. Different types of pathogenic bacteria have been isolated from the pus samples of infected areas in the patients with diabetes. The type of isolates includes pathogenic group of bacteria, anaerobic organisms and other anaerobic non-spore forming Gram positive and Gram negative rods. Among the 50 samples collected, 25 contain, β-haemolytic Staphylococci, 5 contain non-haemolytic Streptococci and 20 samples contain both, β-haemolytic as well as non-haemolytic Staphylococci. The isolated organisms were also confirmed by depend upon their biochemical activities.

Area of residence has been suggested to increase the risk of CVD and CVD specific mortality in the general population but less investigated in adults with type 1 diabetes mellitus.The aim is to deepen knowledge on health inequalities at area level by assessing whether the risk of incident CVD and CVD specific mortality among adults with type 1 diabetes mellitus living in disadvantaged areas differed from those in other areas of Stockholm County. Therefore, a cohort study in which 7544 adults with type 1 diabetes mellitus aged 40-80 years, living in disadvantaged and other areas of Stockholm County in 2006 were followed from 2006-2011. Crude and adjusted hazard ratios for incident CVD and CVD specific mortality were calculated using Cox proportional hazard regression. All models were adjusted for age, gender, education, country of birth, social allowance, disability allowance and disposable income. Results: Adjusted hazard ratios for CVD, stroke, peripheral vascular disease, myocardial infarction and CVD specific mortality were 1.2 (95% CI 0.5-2.0), 1.2 (95% CI 0.6-3.1), 1.0 (95% CI 0.5-2.9), 1.9 (95% CI 1.07-3.6) and 1.3 (95% CI 0.5-4.5) respectively for subjects in disadvantaged areas compared to other areas of Stockholm...

Diabetes mellitus is a disease in the glucose-insulin endocrine metabolic regulatory system, in which the pancreas either does not release insulin or does not properly use insulin to uptake glucose in the plasma, which is referred as hyperglycemia. Complications of diabetes mellitus include retinopathy, nephropathy, peripheral neuropathy and blindness. The purpose of this study is to model the dynamic of the short-term glucose regulatory system (GRS) in man and its hormonal regulation by considering all the feedback loops which are responsible of controlling the glucose metabolism in man. The model proposed in this work used the system dynamics (SD) approach which was designed for studying complex systems. This SD model consists of twenty-three negative (balance) loops and six compartments. The model was validated during wide range of clinical test inputs, to be sure that this model accurately represents the really endocrine system. Understanding the mechanism underlying glucose-insulin oscillations and determination of the origin of the glucose-insulin levels oscillations is another purpose of this model. Other applications for this model also have been explored.

This is a cross sectional study on 400 cases of diabetes mellitus who attended the National Diabetic centre/ AL-Mustensyria university for a period of 6 months. The main factor of metabolic syndrome was dysglycaemia followed by hypertension, obesity and dyslipidaemia. Females in this study were more prone to have metabolic syndrome; Male: Female ratio = 0.5. Those in the age group 60-69 have the highest frequency of metabolic syndrome. Those with BMI 30-40 have the highest frequency of metabolic syndrome. Conclusion: This study concludes that early diagnosis and aggressive treatment is needed for metabolic syndrome to avoid cardiovascular complications. This study is important because of its predictive power for cardiovascular complication and diabetes.
