Thilo Klein Why Do India's Urban Poor Choose to Go Private?. Health Policy Simulations in Slums of Hyderabad

Why does a large proportion of India's urban poor choose to pay for private health services when public services are essentially free of charge? Drawing from previous research, this study takes the underprovision of the medication component in public health packages as a starting point. Field evidence shows that patients in public health facilities often have to get external medication, whereas private hospitals offer a menu of pre-specified care packages. Thilo Klein contributes an answer as to why the poor choose to go private by investigating their risk-aversion and demand for insurance in the choice of health facilities. In discrete choice experiments on maternity care in the slums of Hyderabad, the author finds that the lowest income group attaches significantly higher importance to "full medication" maternity care packages. In line with Expected Utility theory, the study further finds empirical evidence that this insurance demand of the poor is partially explained by their risk-aversion. The findings suggest that an effective pro-poor policy should provide insurance cover for medication-related expenses. They have significant implications for the provision of public health care to the poor and are important for policy-makers and health management alike.

The book focuses on issues and challenges of economic transformation of the peri-urban area around Rajshahi city of Bangladesh. Peri-urban area is the transitional zone between an urban and its nearby rural area. Characteristically, the area is neither urban nor rural but it gradually attains urban characteristics and spatially shifts from urban to rural area. As a result, the economic setting in term of occupation, income, expenditure capacity, land use, land value, infrastructure, living facilities etc. of the dwellers of this area changes rapidly. And this changing process becomes influenced by a number of factors like location, distance from urban center, road network, transportation system, marketing network, employment opportunities, governmental policies and planning and so on. Though, the urban areas have development plan but due to week implementation of plan a haphazard urban growth can be found that reduces the actual competency of the area. So, it is necessary to implement the existing development plan and initiate integrated rural-urban development plan for better, planed and furnished urban development in Bangladesh.

India has experienced rapid urbanization in the past decades. Urban population of the country has increased from 62 million in 1951 to 286 million in 2001. Rapid urbanization is associated with better socioeconomic and health conditions in urban than rural areas due to increased opportunity of education & employment and improved health infrastructures within the cities. On the contrary, the rapid growing urbanizations is characterized as typically overcrowded, polluted, lack basic services such as clean water and sanitation and exposed to infectious diseases. Along with unhygienic and health threatening surroundings, irregular use of recommended health practices, high cost of health facility and accessibility make the health of urban poor vulnerable. Kandappu Mylvaganam The Ethical Case for Clinical Ethics Committees

Modern health care increasingly faces ethical problems in relation to new technologies, costs and rationing and changing social values. An ageing population and an increase in the prevalence of chronic diseases have led to difficult choices in allocation of limited resources. There is greater public awareness of ethical issues through media reporting and a greater demand for public accountability of health care professionals. Health care professionals need not only strive to give the best possible clinical care, but also have to consider the ethical dimensions of the care they provide. To ensure high ethical standards in patient care, individual health care professionals and health care institutions need support and advice. The author argues that clinical ethics committees serve that purpose.

Health services provision remain an essential part of the responsibilities of governments throughout the world in view of the vital role good health plays in economic development. However, in the developing world, the public sector (government) is no longer able to solely provide sustainable basic infrastructure, including health services in response to the rapidly growing demand due to increasing population growth and weak economies. This book used the Upper East Region of Ghana as a case study to examine the contribution of the Catholic Church (a not-for-profit health care provider) to improving access to health care delivery in rural communities. The research used secondary and primary data sources which produced results indicating the immense health care delivery gap being filled by the Catholic Church in rural Ghana. It outlined several dimensions of health care access and identified public-private partnerships as a viable policy for governments to adopt for improved access to health care. This book is useful to government and nongovernmental actors in rural health delivery, persons in academia or anyone with an interest in Church-State partnerships in rural health delivery.

This paper aims to examine the existing right to health recognized in the national legislation of Bangladesh along with the health system and policies. It reviews the right to health mentioned in the international laws, defines case studies and analyzes areas for improvement. This legal case study has been based on literature review from primary and secondary sources and experience gained through personal communication and group discussions. The World Health Organization (WHO) estimates that 10% of the world's population suffers from some sorts of disability. The declaration of the Alma-Ata of "health for all" and MDGs will never be achieved without ensuring health of disabled people. But, unfortunately no significant work has been done in Bangladesh to deliver proper health services to disabled people; rather they are just overlooked by all mainstream services including health. Though there is an abundance of research on health seeking behaviour, very little has been done with disabled people. In order to increase the proportion of disabled people who seek appropriate treatment, health professionals and health programme planners need to know more about factors that influence health seeking behaviour in relation to disability. A better understanding of health seeking behaviour associated with disability should assist us to improve the health services for disabled people. This book is expected to help the readers to understand the health seeking behaviour of disabled people in Bangladesh.

The existence of regional inequalities in health care services is a common phenomenon at global level. Efforts are made to reduce the regional inequalities by promoting equitable access to primary health care (PHC) services and facilities. This study explores the inter-district variation in primary health care services and development in Balochistan. The study uses a set of twelve (12) PHC related indicators, out of them ten (10) are related to curative health care and two (2) indicators are selected covering the curative aspect of primary health care. By using 0-1 transformation value technique, the composite indexes are calculated for curative and preventive health care services. Districts are ranked as under developed, moderately developed, developed and less developed. There are found sharp inter-district variations in curative as well as preventive health care services in Balochistan. In order to promote the primary health care services in Balochistan the proposed recommendations include ensuring equitable provision of health professionals particularly in terms of female doctors, and decentralization of tertiary care services in order to promote primary health care services.

It is internationally recognised by the Universal Declaration of Human Rights and the International Covenant on Economic Social and Cultural rights; both of which Uganda has ratified. This book assesses the RtF situation of slum dwellers of Banda Parish (one of the slums of Kampala City,Uganda)and shows that whereas Uganda is bound by international obligations on the RtF,slum dwellers are yet to realize this right. This is occuring against the background that the RtF is "fairly" protected by objectives XIV and XXII under the social and economic objectives of the 1995 Constitution of the Republic of Uganda. The assessment is based on agreed upon RtF indicators as reported in the project "Measurement Developments in the Realization of the Right to Food by Means of Indicators: The IBSA - Procedure". Recent attention on the role of men in reproductive health warranted the need for becoming of responsible partners in relation to male-involvement initiatives. This study examined the role of men on some selected reproductive health issues. Logistic regression analysis suggests that approval of husband could be an important determinant whether or not a couple would use contraception. Qualitative analysis revealed that a complex web of socio-cultural factors impeded spousal communication regarding the reproductive health issues and discouraged them to take their wives to health facilities. The analysis demonstrates that male involvement in reproductive health is an important strategy to the reach demographic goals in Bangladesh. Men in Bangladesh suffer from misinformation and require factual information from relevant sources to promote and ensure men's participation in reproductive health services. The findings suggest that more pragmatic and target-oriented programmes are required to increase male involvement in reproductive health matters; this will hasten to achieve the demographic objectives of the country.

The global community clamors for American innovation and ideas. But no one wants our health care system.Americans spend trillions of dollars on health care only to generate some of the worst health outcomes in the world. Addressing health care’s problems with incremental solutions cannot generate the transformational change that’s desperately needed. Janis Powers offers a visionary alternative—the Dream Plan.Powers advocates for a system rooted in the values of the American Dream: personal accountability, longitudinal goal-setting, and community support. Her proposal requires a complete decentralization of the current payment system, ostensibly eliminating both health insurance and Medicare while dramatically altering Medicaid. Some of the thought-provoking points in this rigorously researched book include:• Why health insurance will soon be obsolete• Why a longitudinal perspective on health is critical to improving outcomes and saving money• Why the private sector, not the government, must drive health care innovation• Why all Americans must brace for rationing in health care spendingThe Dream Plan elevates the role of preventative care, creates a more market-based economy for health care goods and services, and shifts more accountability for outcomes to the patient. Powers offers an ambitious plan that serves as the first step in the transformation of the American health care system.Janis Powers is a health care strategy consultant who lives in Austin, Texas.

The Bihari community in Bangladesh is deprived of all rights. They are subject to deprivation. They could hardly manage jobs for themselves, which reflected their economic insecurity and food insecurity as well. Inadequate medical facilities in the camps indicated their insecurity of health, thereby becoming vulnerable to various diseases and poor health with debilitating illness. Absence of ownership of land is a violation of human rights. Personal security gets hampered due to identity crisis of the Bihari people because they are not considered citizens of Bangladesh .as well as they do not have any political rights (though court declared their voting right in 2008) to participate in party politics by which they could protect themselves from violation of human rights and other forms of criminal activities. They were excluded from mainstream society.

М. С. Majo Income and health care utilization among the 50+ in Europe and the US

This study addresses the question how income affects health care utilization by the population aged 50 and over in the United States and a number of European countries with varying health care systems. The probabilities that individuals receive several medical services (visits to general practitioner, specialist, dentist, inpatient, or outpatient services) are analyzed separately using probit models. In addition to controls for income and demographic characteristics, controls for health status (both subjective and objective measures of health) are used. We analyze how the relationship between income and health care utilization varies across countries and relate these cross country differences to characteristics of the health care system, i. e., per capita total and public expenditure on health care, gate-keeping for specialist care, and copayments.

Jochen Schmidt Ambulance Response Times in Developing Emergency Health Care Systems

Bachelor Thesis from the year 2006 in the subject Health Science, grade: A, Andrey Jackson University (American College for Prehospital Medicine), course: Emergency Medical Services, language: English, abstract: Dispatcher training can improve ambulance response times in emergency medical service (EMS) systems in low income countries. Emergency health care systems are of increasing concern in international healthcare developments and the global fight against the burdens of disease. Studies show that emergency health care improvements in developing countries should focus on ambulance response. Low-cost changes including emergency medical dispatch training in developing urban emergency health care systems can improve ambulance response times. Concerns about focusing on ambulance response time as a single indicator are addressed by showing a case sensitive approach for developing emergency health care systems that in return can identify ambulance response as main indicator for a particular system. This approach has been chosen for the subject of study, the Osh Ambulance Service, a municipal EMS in Kyrgyzstan. This particular service, the implemented research, the study design and, the data analysis are presented. The data show that there has been a change from an average response time of 23, 18 minutes to 20, 15 minutes. This data is statistically significant and indicates that the implemented changes, despite severe challenges, likely have an effect, but it is unclear if this c...

In the 21st Century, the world will see an unprecedented migration of people moving from rural to urban areas. With global demand for water projected to outstrip supply in the coming decades, cities will likely face water insecurity as a result of climate change and the various impacts of urbanisation. Traditionally, urban water managers have relied on large-scale, supply-side infrastructural projects to meet increased demands for water; however, these projects are environmentally, economically and politically costly. Urban Water Security argues that cities need to transition from supply-side to demand-side management to achieve urban water security. This book provides readers with a series of in-depth case studies of leading developed cities, of differing climates, incomes and lifestyles from around the world, that have used demand management tools to modify the attitudes and behaviour of water users in an attempt to achieve urban water security. Urban Water Security will be of particular interest to town and regional planners, water conservation managers and policymakers, international companies and organisations with large water footprints, environmental and water NGOs, researchers, graduate and undergraduate students. In The Patient Revolution, author Krisa Tailor—a noted expert in health care innovation and management—explores, through the lens of design thinking, how information technology will take health care into the experience economy. In the experience economy, patients will shift to being empowered consumers who are active participants in their own care. Tailor explores this shift by creating a vision for a newly designed health care system that's focused on both sickness and wellness, and is driven by data and analytics. The new system seamlessly integrates health into our daily lives, and delivers care so uniquely personalized that no two people are provided identical treatments. Connected through data, everyone across the health care ecosystem, including clinicians, insurers, and researchers, will be able to meet individuals wherever they are in their health journey to reach the ultimate goal of keeping people healthy. The patient revolution has just begun and an exciting journey awaits us. Praise for the patient revolution «A full 50% of the US population has at least one chronic disease that requires ongoing monitoring and treatment. Our current health care system is woefully inadequate in providing these individuals with the treatment and support they need. This disparity can only be addressed through empowering patients to better care for themselves and giving providers better tools to care for their patients. Both of those solutions will require the development and application of novel technologies. In Krisa Tailor's book The Patient Revolution, a blueprint is articulated for how this could be achieved, culminating in a vision for a learning health system within 10 years.» —Ricky Bloomfield, MD, Director, Mobile Technology Strategy; Assistant Professor, Duke Medicine «In The Patient Revolution, Krisa Tailor astutely points out that 80% of health is impacted by factors outside of the health care system. Amazon unfortunately knows more about our patients than we do. The prescriptive analytics she describes will allow health care providers to use big data to optimize interventions at the level of the individual patient. The use of analytics will allow providers to improve quality, shape care coordination, and contain costs. Advanced analytics will lead to personalized care and ultimately empowered patients!» —Linda Butler, MD, Vice President of Medical Affairs/Chief Medical Officer/Chief Medical Information Officer, Rex Healthcare «The Patient Revolution provides a practical roadmap on how the industry can capture value by making health and care more personalized, anticipatory, and intuitive to patient needs.» —Ash Damle, CEO, Lumiata «Excellent read. For me, health care represents a unique economy—one focused on technology, but requiring a deep understanding of humanity. Ms. Tailor begins the exploration of how we provide care via the concepts of design thinking, asking how we might redesign care with an eye toward changing the experience. She does an excellent job deconstructing this from the patient experience. I look forward to a hopeful follow-up directed at changing the provider culture.» —Alan Pitt, MD, Chief Medical Officer, Avizia «Whether you're a health care provider looking to gain an understanding of the health care landscape, a health data scientist, or a seasoned business pro, you'll come away with a deeper, nuanced understanding of today's evolving health care system with this book. Krisa Tailor ties together—in a comprehensive, unique way—the worlds of health care administration, clinical practice, design thinking, and business strategy and innovation.» —Steven Chan, MD, MBA, University of California, Davis

Contributors include such notables as Tom Daschle, Paul Batalden, and Lord Nigel Crisp. Their commentaries reflect on how it felt to hear the speech in the context in which it was delivered, and assess its relevance in today’s health care environment. The introduction is by Maureen Bisognano, CEO of Institute for Healthcare Improvement, and author of Pursuing the Triple Aim. Praise for previous books by Don Berwick Curing Health Care: «The book is an easy and affirming read for anyone who is familiar with and has used the TQM teachings of Dr. Joseph M. Juran and Dr. W. Edwards Deming and would be a simple and informative introduction to the concepts for anyone who has been hearing about TQM but has no idea what it is all about and wants to know more.» —Permanent Fixes (blog) «Donald Berwick is the most clearly heard evangelist of applying industrial methods of continuous quality improvement in health care.» —Annals of Internal Medicine Escape Fire: «With an effective blend of common sense, real-life stories, persuasive metaphors, and out-of-the-box thinking, Dr. Berwick’s presentations make for fascinating reading for anyone interested in improving America’s $1.7 trillion health care system.» —Piper Report «Anyone interested in change in the healthcare system would enjoy this book. In degree programs, the various speeches would be useful for discussion in a health policy readings course.» —The Annals of Pharmacotherapy

Mohammad Abdul Momin Siddique, Sree Ram Prashad Chakraborty, Mahbuba Akter Ship Scrapping Activities in Bangladesh

Ship breaking is a potential industry for Bangladesh. At present the position of Bangladesh is third in the world for ship breaking activities. This industry pays about 100 million to 130 million USD each year to the government. Bangladesh does not have iron as such has to depend on the scrapped iron and about 90% of iron materials come from this industry. Other items of the ship such as engines, generators, boilers, electrical items, furniture, plumbing items, refrigerators, air conditioners etc. have a good market in Bangladesh. A lot of stakeholders are involved with these scrapping activities. A coherent picture of these scrapping activities in Bangladesh from the angle of socio-economics benefits of different stake holders, constraints and problems for the development of this sector has been drawn in this book.

Jack Wallace Knowledge Transfer and Retention in Health Care - The Implementation of Best Practices in an Acute Care Setting

Over the last few decades advances in health care services have allowed for the effective treatment of many illnesses. The only problem is that proven, effective treatments are not consistently applied in most health care settings. The question is, if we know better, why are we not doing better? This book attempts to answer this question from multiple perspectives. Using a qualitative research design, clinicians from an acute care setting were interviewed about an integrated care pathway for congestive heart failure, two years post-implementation. What follows is an exploration into the impact of system design, organizational culture, and individual will on the eventual disuse of the pathway. As patients begin to demand more consistent care, it will be up to health system leaders to find ways to address these barriers to sustainable knowledge transfer.

Walter Randolph Adams, John Palmer Hawkins Health Care in Maya Guatemala. Confronting Medical Pluralism in a Developing Country

When the traditional meets the modern, nowhere is the impact felt as personally as in the realm of health care. Because practitioners trained in Western science tend to ignore traditional medicine in developing countries, conflict is inevitable.Health Care in Maya Guatemala examines medical systems and institutions in three K'iche' Maya communities to reveal the conflicts between indigenous medical care and the Guatemalan biomedical system. The editors and contributors show how people in this rapidly modernizing society think about traditional practices-and reveal that health conditions in traditional communities deteriorate over time as long-standing medical practices erode in the face of Western encroachment. The contributors first consider cultural, institutional, and behavioral aspects of health care in Guatemala. Then they look closely at the nature and treatment of specific health issues, such as dentistry and mental health-especially depression. Finally they provide new insight on midwifery, nutrition, ethnomedicine, and other topics. As a whole, the volume proposes steps toward a health care system more accessible to Mayas, incorporating K'iche' concepts with Western thought. Representing trends seen throughout the world, it shows the necessity of cultural understanding if poor people are to have access to medicine that combines the best of both local tradition and international biomedicine. Although Western medicine continues to ignore the importance of...

Najmul Haider, Golam Shahi Alam Reproductive Performance of Dairy Cows in Bangladesh

Bangladesh has one of the highest densities of livestock in the world with 145 large ruminants per square kilometer. Dairy is the most important sector of livestock industry in Bangladesh that provides greater part of animal source protein for human consumption. Lifetime productivity of cows largely depends on their individual reproductive performance. In most countries, the primary objective of a reproductive control program in a dairy herd is to have each cow calve and produce a live calf in every 12 months. Like in many other countries, poor reproductive efficiency has been considered to be the major limiting factors in medium and large scale market oriented dairying in Bangladesh for achieving this goal. This book highlighted the overall reproductive efficiency in main milk pocket area of Bangladesh combining both traditional and recent measures of reproductive indexes.

Seminar paper from the year 2014 in the subject Economy - Health Economics, grade: 1,30, University of Kassel (Institut für Volkswirtschaftslehre), course: Social Policy, language: English, abstract: In international comparison, Germany ranks 4th among OECD countries in health expenditure as a share of GDP in 2011. It amounted 11.3% of GDP in 2011 which is 2%-points above the OECD average of 9.3%. Only the United States (17.7%), the Netherlands (11.9%) and France (11.6%) are spending more on health. In addition, German health expenditure increased, in real terms, at a rate of circa 2% p.a. on average in 2000-2010, but slowed down to 1.1% in 2011. However, expenditure continues to grow. With regard to these figures, health care is one of the most important sectors in Germany. With more than 6 million employees and a gross value added of almost € 268 bn in 2013, it represents 11% of the German economy. While expenditure is on the rise, the number of contributors who finance the health care system decreases. As a result, the question of how to finance the growing demand for health services occurs. In particular, a closer look at the statutory health insurance (SHI) is necessary as it financed 57% of total health expenditure in 2011. Since the report "Achieving financial sustainability for the social security systems" by the Rürup-Commission, two major reform proposals are discussed: the citizens' insurance ("Bürgerversicherung"), a universal system of health ...

Christian Funk Law and Practice of Private Health Insurance and Managed Care

Inhaltsangabe:Abstract: A true revolution has taken place in the financing of health care in America. Today, managed care is dominating the way Americans receive and pay for their health care. With the rise of managed care medicine has been wrenched out of its atomized world of solo physician practices and community hospitals and has been transformed into a modern industry of giant for-profit companies traded on Wall Street. The current marketplace is characterized by mergers, acquisitions and the establishment of giant multi-billion dollar healthcare networks. Hospitals and managed care plans run big advertisement campaigns in the media, praising their products and services in order to get the biggest share possible of the $1.1 trillion America spends on health care each year. All parties involved in providing health care lobby for their interests at all levels of political decision-making in order to influence legislators and policymakers. Today’s health care market changes quickly and at a high rate. New variations of managed care arise constantly making any analysis of managed care an ongoing game of "catch-up" with the marketplace. While writing this paper, for example, UnitedHealthcare dropped one of the major managed care instruments, utilization review, to address public’s concerns and pending legislation. This paper will take a “snapshot” of managed care on the eve of the new millennium by using the most recent information available. After this introduction, ...
