Mental health is an integral component of health that helps a person to realize his or her own relational abilities using the psychomotor, affective and cognitive domains. A high prevalence of mental disorders is seen in old age and predominant among these is depression. The community-based mental health studies have revealed that the point prevalence of depressive disorders among the geriatric population in India varies between 13 and 25 per cent. Another common mental health problem of the elderly is dementia. The total prevalence of dementia rises sharply above age 75. Individuals with dementia have reduced survival. The prevalence of cognitive impairment has been estimated between 11% (cognitive impairment- no dementia: ages 65-74) and 98% (memory impairment on at least one objective test). The financial burden including medications, care giving and institutional care; increases with the severity. This book explores the mental health status of the elderly Indian population. It also evaluates the National Mental Health Programme in India and identifies the scope of improvement in the mental health care delivery systems for the elderly.

This timely volume examines the health care systems of Canada, Denmark, France, Germany, Japan, the Netherlands, New Zealand, Sweden, the United Kingdom, and the United States. All democratic and affluent, with well-educated populations and high health standards, these countries have interacted with each other extensively over the years in commercial, cultural, and scientific affairs. The essayists, all resident health care experts, address here the ways in which their countries influence, and are influenced by, the health care systems of other countries studied here. They also examine their common problems-not least the increasing pressure to serve aging populations while maintaining economic balance. Each covering a different country, the chapters detail the various components of national health care systems: the role, choices, and financial responsibility of the patient, physician training and influence, the organization and financing of hospitals, provisions for care of the elderly and mentally ill, public health services, the role of private health insurance, national health expenditures and efforts at cost containment, and the role of government. Each chapter is supplemented with a wealth of statistical data relevant to the respective country. The final chapter by Marshall W. Raffel addresses some of the overarching issues that emerge from the study of these ten countries. While not proposing a solution to all of the problems of health care systems, this volume provides i...

These patients more often than not have greater co-morbidities, more complicated workups and utilize more laboratory and radiologic services. This text is designed to teach emergency physicians how best to care for this specific demographic of patients. It addresses physiologic changes, high-risk conditions, and atypical presentations associated with elderly patients in the ED that result in frequent misdiagnosis or delays in diagnosis. It instructs the readers how best to care for elderly patients in order to minimize morbidity and mortality, addressing some of the difficult psychosocial issues that confront health care providers that care for elderly patients, such as psychiatric disease and end-of-life care. The utility of this text is not limited to emergency physicians, but it should be useful to all health care providers involved in the treatment of elderly patients with acute medical or surgical conditions.

Master's Thesis from the year 2012 in the subject Sociology - Gender Studies, grade: 2:2, National University of Science & Technology Zimbabwe (National University of Science and Technology), course: Masters Degree in Disaster Management, language: English, abstract: This study was carried out against the background of HIV/AIDS having caused an orphan care crisis in Zimbabwe, with the brunt of care falling on the elderly who are themselves weak and frail due to age and socio-economic challenges. This burden fell on mainly the grandmothers who have had to bear the burden of caring for large numbers of orphaned grandchildren, with little or no support from the surviving members of the extended family or other sections of the community and government. They faced a lot of challenges in their new care giving role which included lack of food, lack of money to pay for school fees for the orphans, etc, these challenges added on top of the health problems that the elderly caregivers were enduring with which were age related like BP, Back ache, Arthritis, diminished eye sight etc. Against this background the study sought to determine the major causes of orphanage care, to identify the major challenges faced by these elderly caregivers in taking care of the orphans, and also to establish the coping strategies used by these elderly caregivers in upbringing the orphans and vulnerable children (OVC) under their care. The study also determined the role played by Government and other...

The global community clamors for American innovation and ideas. But no one wants our health care system.Americans spend trillions of dollars on health care only to generate some of the worst health outcomes in the world. Addressing health care’s problems with incremental solutions cannot generate the transformational change that’s desperately needed. Janis Powers offers a visionary alternative—the Dream Plan.Powers advocates for a system rooted in the values of the American Dream: personal accountability, longitudinal goal-setting, and community support. Her proposal requires a complete decentralization of the current payment system, ostensibly eliminating both health insurance and Medicare while dramatically altering Medicaid. Some of the thought-provoking points in this rigorously researched book include:• Why health insurance will soon be obsolete• Why a longitudinal perspective on health is critical to improving outcomes and saving money• Why the private sector, not the government, must drive health care innovation• Why all Americans must brace for rationing in health care spendingThe Dream Plan elevates the role of preventative care, creates a more market-based economy for health care goods and services, and shifts more accountability for outcomes to the patient. Powers offers an ambitious plan that serves as the first step in the transformation of the American health care system.Janis Powers is a health care strategy consultant who lives in Austin, Texas.

The existence of regional inequalities in health care services is a common phenomenon at global level. Efforts are made to reduce the regional inequalities by promoting equitable access to primary health care (PHC) services and facilities. This study explores the inter-district variation in primary health care services and development in Balochistan. The study uses a set of twelve (12) PHC related indicators, out of them ten (10) are related to curative health care and two (2) indicators are selected covering the curative aspect of primary health care. By using 0-1 transformation value technique, the composite indexes are calculated for curative and preventive health care services. Districts are ranked as under developed, moderately developed, developed and less developed. There are found sharp inter-district variations in curative as well as preventive health care services in Balochistan. In order to promote the primary health care services in Balochistan the proposed recommendations include ensuring equitable provision of health professionals particularly in terms of female doctors, and decentralization of tertiary care services in order to promote primary health care services.

This study addresses the question how income affects health care utilization by the population aged 50 and over in the United States and a number of European countries with varying health care systems. The probabilities that individuals receive several medical services (visits to general practitioner, specialist, dentist, inpatient, or outpatient services) are analyzed separately using probit models. In addition to controls for income and demographic characteristics, controls for health status (both subjective and objective measures of health) are used. We analyze how the relationship between income and health care utilization varies across countries and relate these cross country differences to characteristics of the health care system, i. e., per capita total and public expenditure on health care, gate-keeping for specialist care, and copayments.

The effect of health on labor force participation is an established fact. This research hypothesizes the endogeneity of health stemming from the reverse effect, reporting bias and unobserved factors. The relationship between health and labor force participation of elderly Russians is modeled with simultaneous equations using data from World Health Organization Study on Global Ageing and Adult Health, Wave 1 (WHO SAGE, 2007–2010). A strong and non-linear effect of health on labor force participation is confirmed. Unlike for a complex health measure, endogeneity, confirmed by the correlation of unobserved shocks in the system of equations, is found for single-question health measures. The results show that the official retirement age has a moderate effect on labor force participation for women and a weaker or no effect for men. Nevertheless, the health of elderly Russians does not impose strong limitations on the increase of the retirement age.

Modern health care increasingly faces ethical problems in relation to new technologies, costs and rationing and changing social values. An ageing population and an increase in the prevalence of chronic diseases have led to difficult choices in allocation of limited resources. There is greater public awareness of ethical issues through media reporting and a greater demand for public accountability of health care professionals. Health care professionals need not only strive to give the best possible clinical care, but also have to consider the ethical dimensions of the care they provide. To ensure high ethical standards in patient care, individual health care professionals and health care institutions need support and advice. The author argues that clinical ethics committees serve that purpose.

Connected through data, everyone across the health care ecosystem, including clinicians, insurers, and researchers, will be able to meet individuals wherever they are in their health journey to reach the ultimate goal of keeping people healthy. The patient revolution has just begun and an exciting journey awaits us. Praise for the patient revolution «A full 50% of the US population has at least one chronic disease that requires ongoing monitoring and treatment. Our current health care system is woefully inadequate in providing these individuals with the treatment and support they need. This disparity can only be addressed through empowering patients to better care for themselves and giving providers better tools to care for their patients. Both of those solutions will require the development and application of novel technologies. In Krisa Tailor's book The Patient Revolution, a blueprint is articulated for how this could be achieved, culminating in a vision for a learning health system within 10 years.» —Ricky Bloomfield, MD, Director, Mobile Technology Strategy; Assistant Professor, Duke Medicine «In The Patient Revolution, Krisa Tailor astutely points out that 80% of health is impacted by factors outside of the health care system. Amazon unfortunately knows more about our patients than we do. The prescriptive analytics she describes will allow health care providers to use big data to optimize interventions at the level of the individual patient. The use of analytics will allow providers to improve quality, shape care coordination, and contain costs. Advanced analytics will lead to personalized care and ultimately empowered patients!» —Linda Butler, MD, Vice President of Medical Affairs/Chief Medical Officer/Chief Medical Information Officer, Rex Healthcare «The Patient Revolution provides a practical roadmap on how the industry can capture value by making health and care more personalized, anticipatory, and intuitive to patient needs.» —Ash Damle, CEO, Lumiata «Excellent read. For me, health care represents a unique economy—one focused on technology, but requiring a deep understanding of humanity. Ms. Tailor begins the exploration of how we provide care via the concepts of design thinking, asking how we might redesign care with an eye toward changing the experience. She does an excellent job deconstructing this from the patient experience. I look forward to a hopeful follow-up directed at changing the provider culture.» —Alan Pitt, MD, Chief Medical Officer, Avizia «Whether you're a health care provider looking to gain an understanding of the health care landscape, a health data scientist, or a seasoned business pro, you'll come away with a deeper, nuanced understanding of today's evolving health care system with this book. Krisa Tailor ties together—in a comprehensive, unique way—the worlds of health care administration, clinical practice, design thinking, and business strategy and innovation.» —Steven Chan, MD, MBA, University of California, Davis

Written by the President and CEO of the Institute for Healthcare Improvement (IHI) and a leading health care journalist, this groundbreaking book examines how leading organizations in the United States are pursuing the Triple Aim—improving the individual experience of care, improving the health of populations, and reducing the per capita cost of care. Even with major steps forward – including the Affordable Care Act and the creation of the Center for Medicare and Medicaid Innovation – the national health care debate is too often poisoned by negativity. A quieter, more thoughtful, and vastly more constructive conversation continues among health care leaders and professionals throughout the country. Innovative solutions are being designed and implemented at the local level, and countless health care organizations are demonstrating breakthrough remedies to some of the toughest and most expensive challenges in health care. Pursuing the Triple Aim shares compelling stories that are emerging in locations ranging from Pittsburgh to Seattle, from Boston to Oakland, focused on topics including improving quality and lowering costs in primary care; setting challenging goals to control chronic disease with notable outcomes; leveraging employer buying power to improve quality, reduce waste, and drive down cost; paying for care under an innovative contract that compensates for quality rather than quantity; and much more. The authors describe these innovations in detail, and show the way toward a health care system for the nation that improves the experience and quality of care while at the same time controlling costs. As the Triple Aim moves from being largely an aspirational framework to something that communities all across the US can implement and learn from, its potential to become a touchstone for the work ahead has never been greater. Pursuing the Triple Aim lays out the vision, the interventions, and promising examples of success.

Health services provision remain an essential part of the responsibilities of governments throughout the world in view of the vital role good health plays in economic development. However, in the developing world, the public sector (government) is no longer able to solely provide sustainable basic infrastructure, including health services in response to the rapidly growing demand due to increasing population growth and weak economies. This book used the Upper East Region of Ghana as a case study to examine the contribution of the Catholic Church (a not-for-profit health care provider) to improving access to health care delivery in rural communities. The research used secondary and primary data sources which produced results indicating the immense health care delivery gap being filled by the Catholic Church in rural Ghana. It outlined several dimensions of health care access and identified public-private partnerships as a viable policy for governments to adopt for improved access to health care. This book is useful to government and nongovernmental actors in rural health delivery, persons in academia or anyone with an interest in Church-State partnerships in rural health delivery.

Scientific Essay from the year 2017 in the subject Medicine - Alternative Medicine, , language: English, abstract: Integrative health care encompasses measures and activities that aim to achieve an increase of human health resources and the potential of maintaining and improving human health. The term describes the process of empowering individuals to increase their control over determinants of health and thus to positively impact their health. The focus is hereby not only on an individual's behavior, knowledge, and abilities, but also on social, economic, and environmental circumstances. From a holistic perspective, health is defined as physical, psychological, and social wellbeing that is impacted by individual, social, and societal backgrounds. Hence, health is not so much a condition or a goal but rather a resource of daily life. Due to demographic developments, we experience an increase of the population segment with chronic conditions while at the same time less people are paying into the social security system. Chronically ill people not only suffer from their primary disease but also the associated side effects, as in the case of co- and multi-morbidities. A growing shortage of qualified personnel in the service sector and the obscurity of health care structures complicate the access to qualitatively optimized health care. A transformed social framework, reflected in the abolishment of the intergenerational contract and the rise of single households, signifies an ...

With the rise of managed care medicine has been wrenched out of its atomized world of solo physician practices and community hospitals and has been transformed into a modern industry of giant for-profit companies traded on Wall Street. The current marketplace is characterized by mergers, acquisitions and the establishment of giant multi-billion dollar healthcare networks. Promising Care: How We Can Rescue Health Care by Improving It collects 16 speeches given over a period of 10 years by Donald M. Berwick, an internationally acclaimed champion of health care improvement throughout the course of his long and storied career as a physician, health care educator and policy expert, leader of the Institute for Healthcare Improvement (IHI), and administrator of the Centers for Medicare & Medicaid Services. These landmark speeches (including all of Berwick’s speeches delivered at IHI’s annual National Forum on Quality Improvement in Health Care from 2003 to 2012) clearly show why our medical systems don’t reliably contribute to our overall health. As a remedy he offers a vision for making our systems better – safer, more effective, more efficient, and more humane. Each of Berwick’s compelling speeches is preceded by a brief commentary by a prominent figure in health care, policy, or politics who has a unique connection to that particular speech. Contributors include such notables as Tom Daschle, Paul Batalden, and Lord Nigel Crisp. Their commentaries reflect on how it felt to hear the speech in the context in which it was delivered, and assess its relevance in today’s health care environment. The introduction is by Maureen Bisognano, CEO of Institute for Healthcare Improvement, and author of Pursuing the Triple Aim. Berwick, MD, MPP, FRCP, president emeritus and senior fellow, Institute for Healthcare Improvement; former administrator, Centers for Medicare & Medicaid Services «This book is a must for every nurse leader's library and should be a resource and tool in every graduate nursing program, particularly those who are preparing doctoral students focused on systems leadership. Approximately fifty million people in the United States have no health insurance. Dr. Charles Chen, an obstetrician and gynecologist for almost thirty years, agrees with many people in this country that health insurance should cover every citizen, not just those who can afford to pay the rates.A Plan for a Single Payer Health Care System takes an in-depth look at every aspect of our current health care system and provides a strategic plan, along with innovative solutions for reforming the system. In an easy-to-understand format, Dr. Chen provides clear definitions and the pros and cons of utilizing three potential health care systems. Dr. Chen presents the results from his research on several international health care systems and then proposes the best solution for America. He recommends several concrete ideas, including establishing a Department of the National Health Care System, requesting the support of large corporations who pay high medical premiums for employees, and allowing uninsured citizens to purchase a low-cost health plan from the government. The ability to receive medical treatment should be a basic human right. Dr. Chen's plan for improvement helps shine a bright light on a situation that has quickly become critical for this country. His plan will ultimately help ensure that all citizens have access to health insurance.A Plan for a Single-Payer Health Care System takes an in-depth look at every aspect of our current health care system and provides a strategi...

When the traditional meets the modern, nowhere is the impact felt as personally as in the realm of health care. Because practitioners trained in Western science tend to ignore traditional medicine in developing countries, conflict is inevitable.Health Care in Maya Guatemala examines medical systems and institutions in three K'iche' Maya communities to reveal the conflicts between indigenous medical care and the Guatemalan biomedical system. The editors and contributors show how people in this rapidly modernizing society think about traditional practices-and reveal that health conditions in traditional communities deteriorate over time as long-standing medical practices erode in the face of Western encroachment. The contributors first consider cultural, institutional, and behavioral aspects of health care in Guatemala. Then they look closely at the nature and treatment of specific health issues, such as dentistry and mental health-especially depression. Finally they provide new insight on midwifery, nutrition, ethnomedicine, and other topics. As a whole, the volume proposes steps toward a health care system more accessible to Mayas, incorporating K'iche' concepts with Western thought. Representing trends seen throughout the world, it shows the necessity of cultural understanding if poor people are to have access to medicine that combines the best of both local tradition and international biomedicine. Although Western medicine continues to ignore the importance of...

It has become routine to see in various health care magazines, e-mails to leaders, and executive discussions on social media internet outlets, a listing of the ten top challenges that CEOs believe they are facing in the health care industry today. They often call this list the "what is keeping me up at night" list. Although the recognition of their current challenges is important for health system leadership teams and board members, just knowing them is not enough. To be successful in this extremely challenging and complex health care environment today, implementation of action plans is key. The recognition of this critical need to implement a successful road map to the future in order to address the disconnect between knowing and doing has driven the content of this book. Based on the less than positive performance, both clinically and financially, of many health care providers, it seems obvious that the implementation of successful action plans is sorely missing in many boards’ and leadership teams’ processes. The proof that such is the case lies in the fact that the yearly challenge list provided by a wide array of CEOs from small, medium as well as large complex health systems and across the broad health care sector has changed very little over the last decade. We hear and read over and over again about the lack of primary care providers, the declining inpatient volumes, the decrease in profitability, the variability in clinical outcomes, the lack of strong clinic...

Bachelor Thesis from the year 2012 in the subject Medicine - Public Health, grade: 8,4, Maastricht University (Faculty for Health, Medicine and Life Sciences), language: English, abstract: According to Eurostat statistics, the proportion of elderly people in Europe is increasing immensely. While fewer children are born, death is prolonged and people become older. This scenario of demographic changes in the western world raises questions to which answers are rather unclear, as for instance: Who will care for the elderly and how will this look like? The answer to this question may be ambient assisted living (AAL), a branch of information and communication technologies (ICT) aiming at keeping elderly persons as independent as possible in their familiar surroundings in order to postpone hospitalization. The EU plays a major role in developing AAL solutions and enhancing research in the area of ICT, as depicted by the 6th framework programme 2002-2006 or the EU's e-inclusion policy featuring the Digital Agenda 2010, just to name examples (European Commission, 2002; 2007; 2010). However, it remains questionable which challenges AAL - seen as an innovation - faces. The aim of the research in this thesis is to present the field of AAL with relation to home care. Furthermore, the work of the European Union (EU) in the field of AAL is presented, as the EU is a major key player in enhancing research on AAL solutions that help meeting the demands of the future. Moreover, a categorizatio...

In The Patient Revolution, author Krisa Tailor—a noted expert in health care innovation and management—explores, through the lens of design thinking, how information technology will take health care into the experience economy. In the experience economy, patients will shift to being empowered consumers who are active participants in their own care. Tailor explores this shift by creating a vision for a newly designed health care system that's focused on both sickness and wellness, and is driven by data and analytics. The new system seamlessly integrates health into our daily lives, and delivers care so uniquely personalized that no two people are provided identical treatments.
