We performed a retrospective study between December 2010 and May 2011 on all retinitis pigmentosa (RP) cases seen from Okara during ophthalmological consultation at Lyton Rahmatullah Benevolent trust (LRBT) hospital from August 2008 to January 2011. The aim of this study was to determine the preponderance of low vision and blindness in retinitis pigmentosa in patients of Okara. The techniques used for the diagnosis of retinitis pigmentosa were an examination of ocular fundus using a biomicroscope (fundoscopy), far visual acuity and visual field. Indeed, a drop in visual acuity and blindness were the main reasons for consultation in this study. 100 cases were reported of which two were with family history from Okara, 70 male and 30 females were affected from RP. The average age of RP patients was 27.45 with a range of 4-78 years. Blindness and low vision was observed in 54% of the patients and 40% respectively. The average age of RP patients with low vision ranges from 6-50 years and the average age of RP patients with blindness range from 9-65 years. Patients should be screened to enable them to benefit from management focusing on both appropriate treatment and genetic counseling.

Nearly 40% of patients presenting with Non ST-Segment Elevation Myocardial Infarction (NSTEMI) have Chronic kidney disease (CKD).CKD is a powerful predictor of adverse events among NSTEMI patients. CKD patients have a high prevalence of obstructive coronary artery disease. Several studies have been conducted in the past outside Bangladesh but correlation between CKD and outcome in patients with NSTEMI has not been studied in our country. No prior studies have examined the independent association of CKD with coronary anatomy also. Rapid risk stratification is crucial for appropriate management of these patients and for targeting more potent and invasive therapies for higher risk patients. So this study has been designated to evaluate the effect of renal dysfunction on coronary artery disease profile and in-hospital clinical outcome in NSTEMI patients.

Objective: To compare the effectiveness of lumbar stabilization exercises (LSE) with general spinal exercises (GSE) in patients with postero lateral disc herniations. Methods: The randomized controlled trial was conducted A sample of 50 patients was taken from February 2010 to April 2010; and they were randomly placed either in control or experimental group, 25 patients in each group. Results: The progress of all the patients was measured on a unified scale describing 5 disability variables, pain intensity, walking, standing, sleeping and social activity according to modified Oswestry Scale. All participants performed their particular therapeutic exercise with their regular sessions of physical therapy for 4 weeks. By summing up the total of 5 variables, a total pre-exercise and post exercise score were obtained. This total disability score of the two groups were analyzed statistically and t- test was applied which shows statistically significant results with p = 0.000007 Conclusion: Hence patients who performed LSE protocols improved significantly better with lower disability scores compared with patients who had GSE protocols.

Dengue fever (DF) is an emerging mosquito borne viral disease and important public health problem in low land of Terai region which is also expanding to hilly region. This study was designed to estimate sero-prevalence of dengue virus infection in the post monsoon period (Jun-Dec) of 2010 in Nepalese patients with fever visiting hospitals of Birganj, Damouli, Biratnagar and Dhading Besi. Serum samples were collected from 280 patients visiting hospitals with history of fever & clinically suspected DF. The sero-prevalence of dengue virus specific IgM was determined by enzyme linked immunosorbent assay (ELISA) kit (SD, Korea). Molecular diagnosis based on RT-PCR was also performed for the confirmatory diagnosis. The IgM capture ELISA has become the most accepted technique for the diagnosis of dengue in developing countries like Nepal.

This Study shows that HIV prevalence rates are high (20-25%) in Kenya though now dropping,and that variations do exist between regions of Kenya.Adults 15-64 years were estimated at 7.1% or 1.42 million people living with HIV infections in 2007.HIV prevalence among adults aged 15-49 years was 7.4% (KAIS,2007) While prevalence among youths (15-24 years) was found to be 3.8%.In adults aged 50-54 years,5.0% were found to be infected with HIV.Women were found more likely to be infected at 8.8% than men 5.4%. Significant differences in HIV prevalence were found across provinces,among adults 15-64 years in urban areas it was found to be 8.4% and in rural areas it was found to be 6.7%.majority of the infected cases were found in Nyanza and Rift valley provinces. HIV infections has socio-economic and health impacts to individuals/households and to the entire county Government .With support of Donor agencies has put in place Health Programme at AMPATH to support the intake and sustainability of the Health Status of HIV/AIDS Patients.

In order to evaluate smoking status in Macedonian workers’ population, we performed a cross sectional, questionnaire-based study including 753 randomly selected workers. The prevalence of current smokers among all workers was 35.4%, ranging from 30.2% in office workers to 43.5% in construction workers. It did not significantly differ from the prevalence recorded in 2005 (36.8%). The prevalence of ex-smokers among all workers was 10.5%, ranging from 8.4% in construction workers to 12.1% in office workers, and showing minor increase with respect to 2005. The prevalence of passive smokers among all workers was 29.1%, ranging from 26.2% in food processing workers to 32.9% in agricultural workers, remaining similar to their prevalence established in the 2005 study. Our findings indicated high prevalence of current and passive smokers, as well as low prevalence of ex-smokers among examined workers. There were minor changes in their prevalence with respect to the findings from the 2005 study. The results obtained also indicated a need of stricter implementation of anti-smoking activities which will be targeted to all workers.

Although dozens of disabled characters appear in the Grimms’ Children’s and Household Tales, the issue of disability in their collection has remained largely unexplored by scholars. In Disability, Deformity, and Disease in the Grimms’ Fairy Tales, author Ann Schmiesing analyzes various representations of disability in the tales and also shows how the Grimms’ editing (or “prostheticizing”) of their tales over seven editions significantly influenced portrayals of disability and related manifestations of physical difference, both in many individual tales and in the collection overall. Schmiesing begins by exploring instabilities in the Grimms’ conception of the fairy tale as a healthy and robust genre that has nevertheless been damaged and needs to be restored to its organic state. In chapter 2, she extends this argument by examining tales such as “The Three Army Surgeons” and “Brother Lustig” that problematize, against the backdrop of war, characters’ efforts to restore wholeness to the impaired or diseased body. This study represents the pattern of kidney disease and their treatment in tertiary care hospitals in Bangladesh. To conduct the study i consulted the doctors nephrology department of National Institute of Kidney Diseases and Urology hospital to select patients for data collection.120 patients were selected, among which 68 were male and 52 were female. I found that, most of the kidney patient in acute &chronic renal diseases as a public health problem epidemiology, social, and economic implications. The impact of kidney disease on the global burden of diseases is probably underestimated by current methods of evaluation. However, kidney diseases are emerging as a major health problem. I also found that most of kidney patients were present with some other complication, of them hypertension renal disease, retinopathy neuropathy were the most common clinical complication of kidney disease. There is a paucity of data regarding the prevalence and clinical consequences of the chronic renal failure, maintenance dialysis, and renal transplant population in our country. The attendant complications of malaise wasting, anemia, and decreased immunity may predispose these patients to infection.

The difference in the Sino-Nasal-Outcome-20 questionnaire (SNOT-20) score was compared between the COPD cohort and the control group. Secondary outcomes were sub-scores SNOT-Primary Nasal Symptoms (SNOT-PNS), SNOT-Secondary Nasal Symptoms (SNOT-SNS) and SNOT-Quality of Life (SNOT-QOL). Abnormal findings were defined as a value of >12 points and absolute values were compared. Furthermore the Rhino-conjunctivitis Quality of Life questionnaire (RQLQ) and three visual analogue scales (VAS) estimating the amount of nasal secretions, bilateral perception of nasal congestion and nasal itching, were applied. As objective clinical surrogate markers for nasal inflammation, computer tomography (CT) scans of the para-nasal sinuses were performed and assessed with the Newman and the Lund-Mackay scoring system.Results: Data from 83 COPD patients (35 females, mean age: 67 years ± 10) and 34 controls (18 females, mean age: 67 years ± 12) were analysed. The metabolic syndrome a concurrence of disturbed glucose and insulin metabolism, overweight and abdominal fat distribution, dyslipidemia and hypertension, is associated with subsequent development of type 2 diabetes mellitus and cardiovascular disease (CVD). Despite its high prevalence in Asians, no national estimate is currently available on the metabolic syndrome in patients with acute myocardial infarction in Pakistan.A matched case control study using questionnaire and analysis of components of metabolic syndrome was carried out at tertiary care hospitals of Peshawar Pakhtunkhwa.The result showed that patients were 6.19 times more likely to have three or more diagnostic criteria of metabolic syndrome than the controls i.e. 55% vs 17%. This study contributed to the fact that metabolic syndrome is more common in patients with acute myocardial infarction. Screening of adults with central obesity for other parameters of the metabolic syndrome may help to reduce the prevalence of metabolic syndrome as well of myocardial infarction at an early age, which is the complication of metabolic syndrome.

The data about the incidence of rheumatoid arthritis of approximately sixty patients was collected by interviewing them at various hospitals in District Narowal. The data had been analysed to determine the relative occurrence and incidence of rheumatoid arthritis by different age groups. The analysis shows the prevalence of rheumatoid arthritis was more in males (55%, n=33/60) as compared to females (45%, n=27/60). Rheumatoid arthritis was more prevalent in age group 41-50 years (32%, n=19/60) and less prevalent in age group 61-70 years (3%, n=2/60). In our sample population the prevalence of rheumatoid arthritis by age group 11-20 years was 10%, age group 21-30 years was 15%, age group 31-40 years was 28%, age group 41-50 years was 32%, age group 51-60 years was 12% and by age group 61-70 years was 3%. Prevalence is known as the number of all new cases and old cases of a disease during a given time. The prevalence of rheumatic diseases in developing countries is largely unknown. Rheumatoid arthritis is a disease of modern civilization and attacking people in every part of the world and people of every field are being affected.

This book is about the study of association between Helicobacter Pylori (H. pylori) and Chronic Obstructive Pulmonary Disease (COPD). In past years, many studies have been carried out to understand the complex relationship between H. pylori infection and COPD. Few researches have been successful in showing the epidemiologic and serologic evidence for relationship between these two phenomenon. In our study, we aimed to investigate the seroprevalence of Helicobacter pylori in patients with COPD and to determine whether there is an association between H. pylori infection and COPD. The results from our study suggest that there is higher prevalence of H. pylori seropositivity in COPD patients than in non-COPD patients and that HP infection has a positive correlation with the severity of COPD showing the prevalence of HP infection increases with the severeness of COPD. But we were not able to find out the reason for H. pylori infection being related to COPD since our study was carried out in a very small scale and short period of time. To understand the complex relationship between H. pylori infection and COPD, we look forward for a better technical support and a bigger study group.

Diabetes mellitus (DM) is a world-wide endemic with increasing prevalence in both developing and developed countries in a common cause of disability and death all over the world .The outburst of diabetes in the word is mainly due to type 2 diabetes about (90 - 95%). Its specific etiology is not yet known its frequency varies in different racial and ethnic subgroups and is often associated with a strong familial, likely genetic predisposition. It was denoted that type2 DM was associated with certain HLA class II alleles were analyzed for their genotyping by Polymerase Chain Reaction-Sequences Specific Olegionucleotide (PCR-SSO) technique. The present study revealed that diabetic patients were positively associated with HLA-DQB1*0201 (83% vs. 5.0%) which is the most prevalent in patients followed by DRB1*1137 (46.7% vs. 0.0%); DRB1*0401 (41.7% vs. 2.5%),and DRB1*1306(15% vs.0.0%) while HLA,A*0201;B*3559;Cw*0410 and DQB1*0501 is negatively associated in type 2DM in comparison to healthy control groups. This study has shown that there is no significant association between FBS,HbA1c,serum insulin,HOMA2 β-function , HOMA2-IR ,serum adiponectin ,serum adenosine deaminase and HLA alleles .

Human visual system performs major functions when lens focuses light on retina. This light perception is possible due to the physiological and structural system of the eye. Any deformity in the development or structure of lens leads to visual loss. A congenital cataract is a cloudiness of the lens of the eye that a child is born with or develops soon after birth. It may occur due to developmental reasons or from infection, trauma or certain metabolic syndromes that may occur in womb. Between 8.3 to 25% of CC are believed to be inherited. CC have an approximately frequency of 1-15/10,000 live births and report for 8-39% for all the cases. Out of these about one third of CC are familial. Heredity cataracts are shown to account for between 8.3% and 25% of CC. Congenital cataracts are present since birth. A retrospective, analytical study was performed between October 2013 and April 2014 on all congenital cataract cases from Lahore seen during work at the Layton Rahamatullah Benevolent Trust (LRBT) hospital. The aim of this research was to evaluate the prevalence of blindness and nystagmus in patients of CC in areas of Lahore.

Despite the advancement in Total Joint Arthroplasty (TJA) procedures, recent reports indicate that a considerable number of patients (10-30%) continue to report pain, or even worsening of their symptoms, well after their surgery. Although literature suggests that chronic post TJA pain is a common problem, there is limited information about its prevalence among Canadians. This study aimed to investigate the magnitude (prevalence) and categorize chronic pain characteristics (NP vs. Non NP) in the total hip and knee arthroplasty population. A retrospective review of 1143 TJA recipients identified 148 who experienced postoperative chronic pain. Chronic pain was defined as pain ≥3 (out of 5) on the Oxford Pain Questionnaire, 6 months or 1-year post surgery. Participants with chronic pain completed S-LANSS and MPQ (SF-2) questionnaires. Results indicated that 13% experienced chronic pain following TJA; a neuropathic subtype was found in 28% (S-LANSS) - 43% [NP-MPQ (SF-2)], of those. Individuals with chronic pain post TJA with neuropathic subtype reported severe pain intensity and higher disability levels, even 1.5 to 3.5 years post surgery, compared to those without chronic pain.

The data about the prevalence of hypertension of approximately one hundred patients was collected by interviewing them at Sughra Shafih Medical Complex, Narowal. The data had been analysed to determine the relative occurrence and incidence of hypertension by different age groups. The analysis shows the prevalence of hypertension was more in females (64%, n=64/100) as compared to males (34%, n=34/100). Hypertension was more prevalent in age group 38-47 years (32%, n=32/100) and less prevalent in age group 68-77 years (6%, n=6/100). In our sample population the prevalence of hypertension by age group 18-27 years was 13%, age group 28-37 years was 16%, age group 38-47 years was 32%, age group 48-57 years was 18%, age group 58-67 years was 15% and by age group 68-77 years was 6%. Known as “the silent killer”, it may exist for prolonged periods in the individual without symptoms and may manifest only after causing serious irreversible pathology and complications.

Reproductive disorders adversely affect the reproductive effectiveness. Anoestrus, repeat breeding, cystic ovarian disorders have been observed as the most common reproductive problems in cattle. The present investigation was carried out in Bogra district of Bangladesh to evaluate the prevalence of reproductive diseases (RDs)in dairy cows. Out of 1500 cows, 770 clinical reproductive cases were registered from individual questionnaire in four upazilas under the district of Bogra. Genotypes of cows had significant effect (P<0.05) on most RDs. The prevalence of RDs were lower in <3 years age group except anestrus. Body weight had also significant effect (P<0.05) in all RDs. The prevalence of RDs association with body condition, educational qualification of owners of farm, farming experience, floor type had similar effects and the low incidence recorded were in poor body conditions, illiterate farmer, having no farming experience and earthen floor, respectively. Extensive housing system was lowest influences on various RDs of cows at Bogra district. It concluded that, the percentage of prevalence of RDs are higher which are hindering the development of dairy enterprises in Bangladesh.
